You see your doctor and your doctor sends the order to Advanced Sleep Medicine Services. All sleep tests require a doctor's order. We send you a link to our online scheduler and you complete the required forms, review how to use the test kit, and select a shipping date. We ship the home sleep test kit to you and you take the test at home. Our board-certified doctor reviews the data and interprets the results. We send the report to your doctor, who discusses the results and next steps with you.

For PPO and Medicare insurance, your doctor should send the order, progress notes, patient demos and insurance information by fax to (877) 855-6227 or by email to orders@sleepdr.com. For HMO insurance, your doctor should select Advanced Sleep Medicine Services in the medical group authorization portal. If there's a distance filter in the portal, set it to "unlimited" - We serve all of Fontana with mail delivery.
