For PPO and Medicare insurance, your doctor should send the order, progress notes, patient demos and insurance information to us by fax at (877) 855-6227 or by email to orders@sleepdr.com. We handle the authorization process. For HMO insurance, your doctor should select Advanced Sleep Medicine Services in the medical group authorization portal. If there is an address distance limiter, set it to "unlimited" because we ship to all addresses in Kern County. Once the auth is approved, send us a copy by fax at (877) 855-6227 or by email to orders@sleepdr.com.
